The Application Process
Until you officially select which options and deductibles we offer, I will select for you what I would choose for the purpose of a quote. Below
is what my quote will include unless you specify otherwise. But remember, ultimately
the choice is YOURS. For example, you could even select a return of your
premiums rider (at age 65 you will get all your premiums back minus your claims
they paid for). Wouldn't that be great!! Now that's what you call Freedom
to choose. Your policy is customized to meet your unique interests and needs.
However, when you get your quote, the quote will include the coverage in the
area's below unless you requested different.
Depending upon your current health and health history the following options
may be available for you. Keep in mind these options are based upon the company
accepting you.

When we meet, please have all the decision makers available at that time to fill out the paper work
to see if we can get you insured. Allow for approximately 30-45 minutes to review and fill out the necessary paper work and double check to make sure you have the below information ready when we meet.
| Doctors Names, Address, Phone Numbers for Applicants. |
| Any Medications, Name, Dose Amounts and Reasons (for taking maintenance drugs) |
| Last doctor's visit and reason for visit as well as the outcome of the visit. |
| Social Security Numbers for All Applicants. |
| Weight and Height of All Applicants.

| I will also need to know with whom you currently have insurance
and your policy number.

I will need a PERSONAL check to initially set up your application. NO business check will be acceptable, as you will be getting a personal policy.
Your check must include the first month premiums and $115.00 application fee. Full refund will be provided if you do not get accepted or decide you do not want our
protection.
| Any other information I will need for the application will be by asking you questions and checking off
Yes or No. AND then once I have your completed application, first month premium, and $115.00 application fee, I will send it off to our underwriting department,
and hopefully, it won't take too long to get you the coverage and protection you want and need.. Generally, it takes approximately 2-3 weeks to find out if you are approved. If you do get approved, you will have 10 days to decide to keep it
, decline it, or to make any changes. |
